Primary Care Network (PCN) Services

As part of Trafford West Primary Care Network, our practice works with neighbouring GP surgeries to provide additional services that support your health and wellbeing. These services are delivered by a wider team of health professionals, some of whom may be based at a different local practice or in the community.

You may be referred to one of these services by a GP, or in some cases, book directly through our reception team.

First Contact Physiotherapists (MSK)
See a specialist physiotherapist without needing a GP referral — ideal for muscle, joint or back pain.

Dietitians
Personalised advice for managing conditions like diabetes, high blood pressure, digestive issues or weight-related concerns.

Clinical Pharmacists
Expert advice on medicines, reviews for long-term conditions, and help managing complex prescriptions.

Care Coordinators
Extra help for patients with long-term conditions or complex needs — coordinating your care and support.

Social Prescribers
Support with non-medical issues like loneliness, housing, finances or life changes. They connect you to services and activities that can help you feel better.

Children and Young People’s Social Prescriber
Tailored support for young people struggling with social, emotional, or practical challenges — helping them access services, build confidence and improve wellbeing.
